How to make a referral part of routine practice

Teams with the highest referral rates tend to build the referral into a process that they already use.

Some things that clinical teams have told us that work well:

  • Adding referral to your new diagnosis checklist or patient record.
  • Briefing the whole team involved in the blood cancer care pathway and sharing referral form completion reminders.
  • Displaying referral posters and flyers to act as a reminder of the service. Contact us to order printed resources.
  • Some teams offer the referral as part of the holistic needs assessment (HNA) for newly diagnosed patients.

Talking to patients and carers about a referral to Blood Cancer UK

If you're not sure how to describe the referral when offering it to patients or carers, you could say:

  • "Blood Cancer UK offers support and information to anyone diagnosed with blood cancer and their family and friends."
  • "Are you happy for me to pass your details onto Blood Cancer UK, so their support team can contact you with some useful information."
  • "It's fine if you're not sure what you need yet. Blood Cancer UK can tell you about what's available."

Is your hospital listed but you're not referring?

If your hospital listed, a haematology team with your Trust has already completed the on boarding process and the necessary governance approvals are in place.

To get started, read our referral guidance. If you're happy with the process, you can begin referring patients straight away using the referral link.
